UITH School of Nursing Admission Form – The University of Ilorin Teaching Hospital (UITH) is presently accepting applications for the School of Nursing Basic Program for the academic year 2022/2023. Below are the requirements and steps for obtaining a UITH school of nursing form.

UITH School of Nursing Admission Requirements

The following qualifications/features are required of the prospective candidate.

In WASSCE/NECO, you must get five (5) credits in English Language, Mathematics, Physics, Chemistry, and Biology in not more than two (2) sittings.
NABTEB is recognized, although it cannot be used in combination with any other O’ Level certificate (WASSCE/NECO).
As of October 1st, 2022, candidates must be at least 17 years old.
Candidates must not have any criminal convictions on their record.
There must be no deformities in the hands of the candidates.

How to Apply for UITH School of Nursing Admission Form

Fill out the application forms on the University of Ilorin Teaching Hospital website (www.uithilorin.org.ng) and pay a non-refundable fee of ten thousand Naira (N10,000.00) into the University of Ilorin Teaching Hospital account with the CBN using the CBN e-payment portal REMITA.

For your form to be processed, all data and scanned uploads (e.g., certificates, referees, remita receipts, etc.) on the form fields MUST be submitted properly as needed.
Before moving on to the form page, you must first make a payment.
Go to www.remita.net to make a payment.
Select Pay FGN and State TSA from the drop-down menus.
Choose and click FGN Federal Government of Nigeria
Choose the University of Ilorin Teaching Hospital as the recipient of your payment.
Select UITH GENERAL PURPOSE from the Name of Service/Purpose drop-down menu.
Complete the required fields and submit your payment.

You may now complete your registration by filling out the online BN FORM using the Remita Retrieval Reference (RRR) Number obtained with the payment made via the Remita Platform.
When you click APPLY NOW below, you will be sent to an interface where you must input a VALID personal E-mail address.
An email with a confirmation link will be sent to the address you provided during registration.
Open a new tab in the same browser that you used to log into your email account.
Click the link supplied to you by noreply@uithilorin in your e-mail.
You will be able to fill out and submit the BN application form.
Choose the school you want to apply to from the drop-down menu.

Note: You must provide a scanned signature of the sponsor in jpeg, png, or jpg format in the sponsor information form sections.

Note: Each candidate must provide THREE TYPED AND SCANNED REFEREES in jpeg, png, or jpg format, with the REFEREES LETTER HEADED PAPER.

Fill out the rest of the form as required.
Wait 2 seconds after submitting the form to be redirected to print or download the completed form.

NOTE: If you don’t see your printout, it indicates your form was not submitted correctly, and you’ll need to start again.

Note: Please do not bring the printout with you to UITH.

Closing Date: All application forms must be submitted online by Friday, July 29th, 2022.

UITH School of Nursing Entrance Exam & Interview

Written Examination: Between Thursday, 11th and Saturday13th August 2022. From August 1, 2022, candidates must confirm the date on the UITH website.
Oral interview: Monday, 15th August 2022.

VENUE: University of Ilorin Teaching Hospital School Complex (Former Maternity Hospital), Amilegbe, Ilorin.
